Petrelintide meets Phase 2 weight-loss endpoint in people with type 2 diabetes

Published in Drugs development & discovery.

October 8, 2026 — Zealand Pharma reported on October 7 that its investigational amylin analogue petrelintide met the primary endpoint in ZUPREME-2, a randomized, double-blind Phase 2b trial involving 220 adults with overweight or obesity and type 2 diabetes.

At 28 weeks, mean weight loss ranged from 7.4% to 9.2% across three weekly injection arms, versus 2.0% with placebo, using the efficacy estimand. All three comparisons were statistically significant, according to the company. Participants also received diet and activity measures and metformin, with or without an SGLT2 inhibitor.

The largest reported HbA1c decrease was 0.65 percentage points, compared with a 0.23-point increase on placebo. Gastrointestinal adverse events led to discontinuation in 1.9% of petrelintide recipients versus 1.7% on placebo; most reported gastrointestinal events were mild.

These are company-reported topline results, with full findings due at a scientific conference. Petrelintide remains investigational. Longer trials must establish durability and safety; the results do not establish superiority to other weight-loss medicines.
