Remigromig: BRUNELLO reports one-year vision results and a retinal safety difference
Published in Diseases!, Drugs development & discovery.
Published October 11, 2026. Merck presented one-year BRUNELLO results on October 10 for investigational remigromig, an antibody activating the Wnt pathway, in diabetic macular edema.
The randomized, double-masked Phase 2b/3 study enrolled 984 participants. Mean visual-acuity gains were 9.1 letters with remigromig 0.5 mg, 8.7 with 0.8 mg, and 11.8 with ranibizumab. Both remigromig doses met the prespecified non-inferiority criterion; no secondary endpoint demonstrated superiority to ranibizumab.
Safety findings warrant attention. Events associated with proliferative diabetic retinopathy occurred in 6.7% and 6.1% of the remigromig groups versus 0.9% with ranibizumab. Discontinuation because of adverse events was also more frequent: 4.9% and 4.5% versus 0.9%.
Interpretation: Non-inferiority within a predefined margin does not mean identical outcomes or greater benefit. These sponsor-reported conference findings support further evaluation while highlighting a safety difference. Remigromig remains investigational, and additional analyses are underway.
