Large screening study compares AI-assisted cytology and HPV testing strategies

A Nature Communications study compares cervical screening strategies using records from 284,002 women screened in Wuhan, China. It evaluates artificial intelligence-assisted liquid-based cytology alongside high-risk HPV testing.

The researchers modeled nine strategies, comparing detection of CIN2 or worse—precancerous changes or more serious disease—with referral for colposcopy. After correction for verification bias, HPV testing alone had higher sensitivity than AI-assisted cytology alone, but also a higher referral rate.

HPV genotyping followed by AI-assisted cytology achieved 92.68% sensitivity with a 3.34% referral rate. A combined testing strategy achieved 96.37% sensitivity with a 3.70% referral rate under the study’s definitions.

The findings illustrate trade-offs between detecting lesions and referring patients for further investigation. They do not show that AI alone is superior to HPV testing, nor do they establish reduced cancer mortality. These were hypothesized strategies evaluated in a particular screening population; wider implementation requires validation and attention to local resources.
